Few notes for exam:

ACE inhibitor can cause dry irritating cough, caused by elevated pulmonary bradykinin, it is worse during night time


ACE inhibitors may reduce blood glucose and may account for cases of hypo-glycaemia-related admissions.


In renal artery stenosis, the afferent pressure is reduced by the narrowed vessel; hence autoregulation is almost exclusively dependent on changes in post-glomerular arteriolar tone through renin-angiotensin-aldosterone (RAAS) system.
ACE inhibitors interfere with RAAS so glomerular perfusion falls, renal ischemic nephropathy develops and renal failure occur.


In the case of infective endocarditis, prolongation of PR interval means that the myocardium has been affected and there may be formation of an abscess, surgery may needed.


Several lung disorders can cause SIADH by unknown mechanisms. Pulmonary diseases causing SIADH including pneumonia, bronchial asthma, atelectasis, acute respiratory failure and pneumothorax.


In case of hypothyroidism there is low free T4 this leads to increase in TRH which also raises prolactin levels and thus can lead to amenorrhea.
